Calls getting disconnected abruptly while trying to transfer

When I attempt to transfer a call sometimes it's disconnected abruptly. What gives?

We've seen this happen when callers are attempting a loop, meaning you are transferring a call back to someone who transferred your call originally. This scenario is not supported:

Person A calls Person B (This is A’s call)
Person B then transfers Person A to Person C (This is B’s call)
Now A and C are in call (This is the incorrect assumption. While Person A and Person C have bridged audio, Person A is still in a call with Person B from A’s reference point.)
If Person A attempts a blind transfer call to Person B (this is, in effect, A transferring B to B, creating a feedback loop) the call will be dropped.
